Your Hospice Team
We believe that the key to exceptional patient care is our highly skilled and compassionate staff. The Monarch team carefully creates a plan of care to meet your goals, preferences and unique needs.
Nurses
Registered Nurses (RNs) manage and coordinate the care and services that your physician orders for you. An RN is available 24 hours a day, 7 days a week. Licensed Practical Nurses (LPNs) may make visits to assist with your care as well.
Physicians
Working with your primary care Physician, a hospice medical director is available to assist with pain and symptom management and to oversee your care.
Social Worker
Social Workers provide counseling and assistance with social and financial issues relating to your medical condition. Social Workers provide emotional support and can help connect you to community resources.
Spiritual Counselors
A spiritual counselor, or chaplain, can support you and your family with spiritual needs as well as coordinate support between you and community clergy as well.
Hospice Aides
Hospice Aides provide personal care under the supervision of a Registered Nurse to help assist you in activities such as bathing, dressing, grooming, and walking. Services will be provided as determined by an RN’s assessment of patient and family needs.
Therapists
Licensed Therapists can provide services and assistance when needed. These services may include Physical, Occupational, and Speech therapies. Your attending physician will order therapy for you if it is appropriate.
Bereavement Support
Monarch provides team members to assist you and your family in addressing issues of grief and loss. These services are available to you and your family as long as needed, in the present and in the future.
Volunteers
Volunteers are available to support and assist you and your family, and are available for special tasks and companionship.
Nurse Practitioner
Working with our medical director, a nurse practitioner can provide face to face visits, when deemed necessary
